Adoption Agreement... A form will be provided to you upon selection of your pet. 1. I am 18 yrs of age, or older. I have carefully read, agree with and fully understand the terms and conditions of this contract. 2. I will provide the animal I am adopting, from here on referred to as (said animal), with kind, loving care, including yearly Vet visits, and recommendations, vaccinations, worm medications and current identification. 3. I will not sell, abandon, or give the said animal away, nor will I allow it to be used for religious sacrificial purposes, blood sports (dog fighting) or experimentation. 4. I will not allow the said animal to run loose or become a public nuisance, and will immediately retrieve it, should it end up in any shelter. If lost, I will call any local shelters, Vet Clinics and alert the local authorities, making every effort to find the said animal. 5. I will make every effort to accommodate the said animal to my home, spending adequate time to help make the transition to the new environment, a happy, positive experience. I also agree to educate myself about training the said animal. I will contact SECARE and other professionals if I need advice. If during the six (6) month period following adoption of said animal, my efforts are unsuccessful, and I cannot keep the said animal, I understand and agree to contact SECARE. A determinatino may be made at that time, or an assessment scheduled to determine acceptability of said animal for return to SECARE. Circumstances that may prevent an animal from returning to SECARE include (but are not limited to): length of time the owner has had the animal and arising issues such as behavioral or medical that have developed over that time. Determination of acceptability is at the sole discretion of SECARE. For an animal adopted within the six (6) month time period following adoption: I understand that I will not be reimbursed for any funds, I do have the option of adopting another animal (within that same 6 month period from the date of the adoption), without charge (unless SECARE has incurred an unusual expense). 6. I understand that Colorado state law requires all animals adopted from any shelter be altered. If my said animal is not yet altered, I understand that I must have it altered within ninety (90) days of the adoption date unless a written exception or postponement concerning risk to the animal's health can be provided to SECARE by adopter's vet. A deposit of $50 (included in the adoption fee) will be set aside to be used for the spay or neuter only of said animal. If adopter opts to use a vet other than the vets/clinics listed with SECARE, the deposit will be refunded once proof of spay/neuter (verified by the vet) is provided to SECARE. I UNDERSTAND THAT NOT COMPLYING WITH ALTERING IS IN VIOLATION OF STATE LAW AND WILL RESULT IN SECARE RECLAIMNING THE SAID ANIMAL. 7. I accept the said animal as is and assume all risks of its ownership, including the risk of injury or damage caused by the animal such as (dog bites, accidental fights and destruction of property), on behalf of myself, my heirs, personal representatives and assigns. I hereby release, discharge, indemnify, and hold harmless SECARE, it's directors, volunteers and agents from any claims, causes of action, and demands of any nature, whether known or unknown, arising out of or in connection with my adoption, care or ownership of said animal, including and without limitation any treatment or handling of the said animal by a Veterinarian, or other provider recommended by SECARE. This contact may be specifically enforceable by SECARE through judicial proceedings in the event that I Breach this contract and SECARE files suit to enforce its terms and/or to defend any claims hereunder. I will pay any and all costs incurred by a court action I cause in connection therewith. ABOVE ALL THE ADOPTER PROMISES TO ALWAYS TREAT THE SAID ANIMAL WITH KINDNESS, COMPASSION, AND LIVE UP TO T HE DEFINITION OF HUMANE: MERCIFUL, SHOWING KINDNESS TO THE HELPLESS. |
